The Midtown team includes registered nurses, masters-level exercise physiologists, certified medical fitness specialists, corrective exercise specialists, and wellness coaches. We serve as an extension of a patient’s care team and partner to reduce risk factors, improve post-discharge outcomes, and provide guidance, tools and wellness resources to stay healthy for the long term.

Situation

Our health system client’s fitness center lacked medically based programming. They wanted a partner to create a program that could vastly improve its rehabilitation capabilities and make it more convenient for patients to stay close to the system.

Action

Partnering with their system’s Chief Medical Officer and their cardiology and occupational health service line leaders, we developed a series of dynamic, patient-centered fitness and rehabilitation programs. Each program—varying in length and specialty focus—introduced patients to movement, nutrition and mindfulness in an educational, interactive and fun environment. Primary care physicians and specialists now prescribe these programs through the hospital’s EHR to reduce patient risk factors and enhance post-discharge outcomes.
